Patientree does not use the label “RPM” in the UI. Functionally, RPM maps to monitoring, health metrics, and symptom tracking between visits.

Step 1 — Define your RPM program
Decide:
- Which conditions you monitor (heart failure, hypertension, diabetes, etc.).
- Which devices you support (BP cuff, glucometer, pulse ox, etc.).
- Who reviews readings (RN vs MA).
- Escalation paths for out-of-range values.
Document this in a one-page protocol before enrolling patients.
Step 2 — Enroll the patient
- Obtain informed consent for RPM (device use, data transmission, billing).
- Complete 99453 setup: teach the patient to use the device, verify first successful reading.
- Link the patient in Chronic Disease → Monitoring (
/dashboard/chronic-disease/monitoring).
Step 3 — Patient-side data capture
Patients can view and contribute data in the portal:

Staff should verify patients understand how often to take readings and what to do when values are abnormal.
Step 4 — Staff review queue
- Open Monitoring on the staff dashboard.
- Review trends and out-of-range readings in monitoring charts.
- Document interactive communication when you call, message, or video chat about readings.
- Log minutes contemporaneously for 99457 / 99458.
Step 5 — Billing readiness
Monthly checklist:
- 16+ days of transmission for 99454 (when applicable)
- Interactive communication time documented
- Clinical escalation noted when readings required intervention
- Device period aligned with claim month

- Assign a single RPM nurse reviewer per panel week.
- Batch routine reviews; escalate abnormal readings same day.
- Pair RPM with CCM when patients need both — but do not double-count the same minutes.
